I ran an underwater combat recently and for the most part i just went with the rules as discussed in underwater combat on phb p198 and under suffocating on phb p183. Finding ways to both use the 3d nature of the environment to enhance storylines and ways to properly represent underwater combat. This has included things like a warehouses near the docks that have a hole in the floor into an underwater cave because their trading partner isn t across the water but underneath it and figuring out how to use.
